This is the contributors release form we had to fill out in order for people to know who was in my photo-shoot, and information on them. Had to state address and phone number and officially sign this document.

People At Risk Sheet

This was the people at risk sheet we had to fill in. We had to fill this in and say what we would have to look out for before, during and after the production. This was to stay safe during the photo-shoot.

Risk Assessment Sheet

This is my risk assessment sheet i had to fill in before we done the photo-shoot, we had to list the dangers in our shoot and what we had to be careful of during the production. This would help us to stay safe throughout.
